At Forum Dentistry, our referral program makes it easier than ever to give and get great dental health.
What is the referral program?
Our referral program provides a $25 discount* for both the referrer and the referred! It's our way of thanking our valued patients for spreading the word about our exceptional dental care.
How our referral program works
Getting started is easy! Simply follow these steps:
- Tell your friends and family about us: Refer your favourite people. If you love visiting us for all of your dental needs, then it's likely your friends will love us too!
- They just need to let us know that you referred them: All they need to do is mention your name to our reception team, and they'll receive $25 off* off their first appointment.
- After their first visit, you’ll get $25* off: We'll send you an email or SMS confirmation for $25 off your next appointment.
There’s no limit to how many people you can refer, so why not reach out to friends and family today? Whether you chat, call, email, or text them - spread the word and enjoy the rewards!
Terms and conditions
- The $25 will only be taken off the referrer and referred patient’s respective out-of-pocket expense, not their health fund rebate (where applicable).
- Referrer must be an existing patient of Forum Dentistry, and over the age of 18.
- The $25 discount cannot be exchanged for cash and it can only be redeemed once per visit.
- The referred patient can claim their $25 discount at their first appointment, as long as they notify the participating practice at their first appointment, who their referrer was.
- After the referred patient notifies Forum Dentistry of the referrer’s details as set out in (4) above, the referrer will receive an email or SMS confirmation to enable them to claim the $25 discount at the referrer’s next visit to a participating practice.
- There is no limit to the number of patients that each referrer can refer to Forum Dentistry.
